Aetna, AZ Foundation, BC/BS, Cigna, Great West Healthcare, Healthnet, Humana, First Health, Medicare, Pacificare, United Healthcare, AHCCCES and most all insurances are accepted.

Pre-Appointment Forms

To expedite your office visit, all forms are available below. You have the option of both PDF or Word documents. Should you wish to print out the forms, complete them by hand and bring them with you to your visit, select the PDF options. If you prefer to complete them online, save them to your PC, then attach them to an email to our practice (sonoranallergy@gmail.com), select the Word option.

Allergy Injection Hours

Tuesday 9 a.m. – 6:00 p.m. (through the lunch hour)
Wednesday 1p.m. - 6 p.m.
Friday 8.30 a.m. – 5:30 p.m. (through the lunch hour)
